Job Description
- Conduct comprehensive assessments of residents' health conditions and develop individualized care plans.
- Monitor and evaluate the effectiveness of care plans and adjust as needed to ensure optimal outcomes.
- Administer medications and treatments in accordance with physician orders and nursing standards.
- Provide specialized care for residents with chronic conditions, dementia, or palliative care needs.
- Act as a clinical resource for nursing staff, providing guidance and support to improve skills and care quality.
- Oversee the implementation of evidence-based practices and maintain high clinical standards.
- Participate in quality improvement initiatives and audits to enhance care delivery.
Requirements
- Min Diploma / Degree in nursing
- Min 5 years of experience in similar settings
