Job Summary:
- Conduct outreach and stakeholders’ engagement.
- Perform admission and financial counselling.
- Provide the full spectrum of case/care management service to patients and their caregivers.
- Support the clinical team in provision of care to the patients and caregivers
Roles and Responsibilities:
- Engage partners to solicit referrals and stakeholders’ management.
- Manage patients and caregivers through assessment, development of care plan, execute the intervention plan (Deliver service internally or matching patients to the appropriate services), track and monitor the progress of patients and caregivers.
- Facilitates patients' placement through admission and financial counselling
- Familiarize with patient treatment cost and additional costing if there is.
- Familiarize and understand the patients’ financial resources such as subsidies, insurance, Medisave/MediShield, etc.
- Conduct visits to dialysis centre, home or hospitals to provide assessment and supportive counselling
- Advocate for the protection of patient's welfare and rights.
- Collaborate with internal and external healthcare professionals for a more complete patient management
- Liaise with government, community, social agencies or individuals to mobilise social resources for the best interest of patients and caregivers
- Facilitate patient and caregiver support group programmes
- Review and enhance processes to ensure that they are relevant to the needs of patients and caregivers.
- Facilitate discharge and Transfer
- Any other tasks or duties that your head of department assigns to you.
