Responsibilities
- Interviews and counsels patients and their families to help them with their psycho-social problems.
- Communicates with medical staff, nurses, other health-care professionals to discuss plans for total patient management.
- Liaises with other government departments, social services organisations or private agencies to meet patients' needs.
- Maintains case records on patients.
- Participates in the orientation of new staff.
- Represents department on committees of statutory, voluntary and community organisation providing social services.
- Attends supervision session, department meetings, case conference and training workshops and seminars.
- Carries out psycho-social research and relevant special projects.
- Visits patients' homes to assess social conditions and family dynamics, counsels and gives support.
Requirements
- Degree in Social Work or equivalent, preferably with Honours degree or Masters.
- Preferably a Registered Social Worker (RSW) with Social Work Accreditation Board.
- Prior experience in social work or the social agency setting is preferred.
- Good counselling and communication skills.
