As a Medical Social Worker, you will be responsible for the following duties:
- Providing counselling to patients
- Helping the patient to apply for financial help and connecting with community resources.
- Teaching the patient and next of kin on the government policies and navigating medical systems
- Conducting of psychosocial and financial assessments.
- Making home visits to patients and their families.
- Conducting of training for caregivers and internal staff.
- Handling case management.
Requirements:
- Min Degree in Social Work
- At least 1 year of experience is preferred
- Preferably a registered Social Worker with the Singapore Association of Social Workers (SASW)Proficient with Microsoft Office
- Effective Oral and Written Communication Skills
