Summary:
• Public Healthcare Institute (central region)
• Work Days/ Hours: 5 Days, office hours
• Contract till 30 Sep 2025
Responsibilities:
• Conduct initial consultation with participants using data gathered from a digital health platform
• Use data to track participant progress and adjust plans accordingly
• Create and implement personalized health plans that are aligned with lifestyle goals set by GPs
• Offer continuous support and guidance to participants through virtual coaching sessions and deliver personalized prompts and reminders to keep participants engaged and on track
• Stay up-to-date with the latest digital health technologies and trends
Requirements:
• 1-3 years of experience in healthcare sector is a preferred!
• Diploma equivalent professional qualifications in Nursing, Social Work, Allied Health, Health Services Management, Health Management & Promotion etc.
• Strong communication and interpersonal skills, able to work independently in a fast-paced environment
• Strong team-player, with natural ability to interact with clients and/ or families
