- Day Care Centre (west region)
Job Responsibilities
- Build rapport with residents/ clients and their families
- Maintain communication with clients while addressing their concerns and goals
- Provide casework management for clients and coordination with hospitals/ polyclinics for clients who require financial assistance
- Develop and implement individual care plans for clients and provide therapeutic support as needed
- Financial assessment and fund management and prepare periodic reports as required - identifying and referring cases for financial assistance
Requirements
- Degree in social work or relevant
- Great interpersonal and communication skills
- Experience working with elderly is a plus!
