Responsibilities
- Screen and recruit patients for inpatient and outpatient Cardiac Rehabilitation Programme (CRP).
- Provide education and counseling to patients and families on disease management, lifestyle changes, and rehabilitation plans.
- Coordinate patient appointments, referrals, and follow-ups with multidisciplinary teams.
- Manage CRP operations, including data entry, supply requisition, and maintaining patient records.
- Conduct nurse-led education sessions and assist in developing patient education materials.
- Act as a resource for staff on cardiac rehabilitation matters.
- Support hospital events, quality improvement, and research projects.
- Perform administrative and operational tasks to ensure seamless service delivery.
Requirements
- Possess at least Degree in Nursing
- Registered Nurse with minimum 3 years of Acute/Cardiac background has added advantage.
- Good interpersonal and communication skills.
- Competent in IT skills
- Team player who enjoys working in fast pace environment.
