Home Care Nurse - Chronic Illness & Palliative Care
Position: Home Care Nurse
Salary: $3,000 - $5,200
Working Hours: Monday to Friday, Office Hours
On Call: One Saturday every 2 months (with a day off in lieu)
We are looking for dedicated and compassionate Home Care Nurses to join our team and provide essential care to chronic ill and palliative patients at home.
Key Responsibilities:
- Perform home visits to assess and manage chronic ill/palliative patients.
- Conduct nursing assessments and coordinate with doctors for medical evaluations and follow-up visits.
- Monitor patient medication adherence and compliance with doctor-prescribed treatments.
- Perform critical nursing procedures such as wound dressing, catheter insertion, naso-gastric tube insertion, and care of medical tubing.
- Develop and implement nursing care plans and suggest new measures for patient management.
- Educate caregivers/families on essential nursing procedures to ensure continuous care at home.
Requirements:
- Minimum Diploma/Degree in Nursing.
- At least 2 years of specialized training in Onco, Home Care, Palliative Care, Geriatrics, or IPW.
- Ability to work independently with minimal supervision.
- Experience in managing 3-5 cases per day on average
