- To perform home visits to chronic ill/ palliative patients
- To perform nursing assessment on the patient
- To coordinate with doctor for medical assessment and follow-up visits to patient
- To monitor current medication and compliance as prescribed by the doctor
- To perform nursing procedures such as wound dressing, insertion of urine catheter, insertion of naso-gastric tube and care of tubings as required
- To initiate nursing care plan and implement new measures of management
- To educate caregivers/families on basic nursing procedures to ensure continuation of care at home
Requirements
- Min. Diploma / Degree in Nursing
- Min 2 years of Onco trained / Home Care trained / Palliative care trained / Geriatric trained / IPW trained
$3K to $5.2K
Working Hours : Office Hours, Monday to Friday
On Call : One day every 2 months, usually on Saturday : Will be given one day off in lieu
Background :
- First month will have buddy attachment then will be on their own. Need someone that’s independent enough to work on their own
- Average one day they will be seeing 3-5 cases.
