Job Summary
- Part of the centralised referral team that is responsible for:
- Management of referrals for all Senior Care Centres (SCCs),
- Implementation of Admission processes centrally and at SCCs,
- Support other functions of Company's HQ where necessary.
Job Description
- Liaise with referral Source/ Agency for referrals related matters such as obtaining sufficient information (past medical history, social reports etc) for referral screening, other contacts for non responsive contacts.
- Screen referrals for right siting of services according to the criteria
- Contact clients or caregivers to clarify services and needs which includes financial assistance, transport, escort etc.
- Schedule appointments for admission assessment accordingly and update centres of the referral status.
- Conduct pre-admission counseling which includes explaining the service terms and fees to the clients or caregivers, clarifying their concerns if any.
- Raise referrals to other service providers.
- Provide reports to the management on referrals status and centre vacancies for monitoring and reporting purposes.
- Support the onboarding of new staff in centre-based administrative processes.
- Support other functions of Company's HQ where necessary.
- Carry out other responsibilities and duties as assigned from time to time, e.g. coverage duties at centres and HQ.
Requirements
- Diploma in Health Managment and Promotion or relevant discipline
- At least 3 years of relevant working experience in the healthcare industry or good understanding of health-care related services.
Working Hours
Monday - Friday: 830am - 6pm
