Roles and Responsibilities
- Assist the nursing team in providing personalized and cost-effective care.
- Monitor and report patient conditions to senior nurses.
- Help with discharge planning and post-discharge care coordination.
- Educate patients and caregivers on self-care management.
- Conduct and document nursing assessments.
- Develop individual care plans based on patient needs.
- Provide necessary nursing services (e.g., feeding tube, catheter, dressing changes).
- Ensure safety and infection control during home visits.
- Collect payments from clients and submit to the Finance department.
- Conduct initial assessments for additional services (e.g., financial assistance, mobility fund).
- Mentor and supervise new and junior nurses.
- Perform safety checks on home care equipment.
- Assist with administrative tasks as directed by senior staff.
- Help handle complaints and assist in service recovery.
- Contribute to professional development through Continuing Professional Education (CPE).
