Job Description:
- Conduct home visits to deliver psychoeducation, support, and care coordination services to clients and caregivers.
- Assess patients' medical, psychological, social, environmental, and vocational needs.
- Collaborate effectively with a multidisciplinary team, including allied health professionals, doctors, and nurses.
- Provide psychological expertise during multidisciplinary meetings.
- Facilitate training sessions for team members and caregivers as needed.
- Execute projects and responsibilities assigned by the supervisor or Head of Department.
Job Requirements: Min Degree in Nursing / Psychology / Social Work / Counselling / Allied Health or similar w min 3 yrs of relevant experience, preferably in psychiatric settings
