Location: Yishun/ AMK
Job Scope:
• Perform relevant clinical assessment and evaluate the patient’s current disease control.
• Assess patient’s concerns about his/her condition and learning needs.
• Explore for possible precipitating factors for sub-optimal/poor control.
• Reinforce patient/caregiver’s education and self-management skills.
• Develop/evaluate the plan of care to improve disease control with patient/caregiver.
Job Requirements:
- Min. Diploma/ Degree in Nursing
- Possess Advanced/ Specialist Diploma in either Chronic Disease/ Mental Health/ Endorcrine/ Diabetes/ Gerontology
