The executive will support the Population Health Management (PHM) leadership in developing and maintaining comprehensive population health profiles. Their contributions will be essential to improve care quality, outcomes, equity, and optimise system value for defined patient populations and residents.
The post holder will support the team in the following areas:
- Drive population profiling and segmentation to inform system planning and care delivery to become more systematic and targeted, and
- Effective use of population health data to draw insights for strategic planning
Job Requirements
- Bachelor in Mathematics, Public Health, Life Sciences or Statistics would be preferred.
- Relevant experience in population health, healthcare planning, product management or program management roles preferred
- Working knowledge of Microsoft Office is required and experience in data analytics and visualization tools will be an advantage.
- Strong written, analytical and communication skills;
