Role: Assistant Executive (Care Coordination)

Salary: $2,800 - $4,000
Requirements:
- Minimum Diploma qualification
- Relevant experience in Senior PSA / Case Management / Care Coordination
Responsibilities
- Referral Management:Liaise with referral sources/agencies to obtain detailed information (e.g., past medical history, social reports).
Follow up with non-responsive contacts.
- Screening and Assessment:Screen referrals to ensure the right siting of services according to criteria.
Contact clients or caregivers to clarify needs, including financial assistance, transport, or escort requirements.
Schedule admission assessments and update centers on referral status.
- Pre-Admission Counseling:Explain service terms, fees, and address concerns of clients or caregivers.
- Reporting:Provide management with referral status updates and monitor center vacancies.
- Coordination:Raise referrals to other service providers as needed.
Assist with onboarding new staff in center-based administrative processes.
Carry out additional duties such as providing coverage at centers and HQ.
