Job Scope:
- This role will be abit more towards case management – manage the care plan for the patients
- You will be the key person who will make the most difference as you make home visits to provide palliative care and recommend psycho-social support to our terminally ill patients and their families.
- You will be part of the most dedicated, compassionate and caring teams comprising doctors, nurses and medical social workers providing an all-in-one professional care to patients at home at all hours.
The Job Requirements:
- Diploma / Degree in Nursing
- Registered with Singapore Nursing Board
- Valid BCLS
- Venepuncture skills is a plus
