RESPONSIBILITIES
Join us as a Case Executive and be part of our CREST team.
You will need to
- Provide care coordination and service linkage to clients with or who are at risk of mental health issues and/or dementia, with the appropriate resources and support including caregiver support
- Plan and conduct outreach to the community and stakeholders including faith-based,corporate and community organisations to promote mental wellness and mental health awareness
- Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support
- Plan and organize structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients
- Conduct regular visits to monitor the well-being of the elderly client (NTUCH CCMS/CREST)
- Support the evaluation and documentation of the programme
- Work with various organisations (i.e. regional health services, hospitals, volunteers, grassroots schools, and internal organisation departments) on activities and programmes for the seniors
- Support other ad-hoc assignments at the discretion of the CREST Lead/Department head
- Diploma/NITEC in community care & social services (without working experience)
- Minimum 3 years experience in community healthcare settings in Singapore
- Possess 1-2 years of administrative experience
You should
- Have good knowledge of the geriatrics and community services
- Be proficient in Google Suite applications
- Have strong communication and stakeholder management skills
- Be passionate about community services and be service-oriented
- Have good assessment and decision-making skills
- Be meticulous and enjoy working independently
Advantageous skills:
- Ability to converse in local languages
