As a Home Care Nurse, you will be responsible for the following duties:
- Evaluate patients' conditions and implement appropriate nursing care plans.
- Monitor and record patients' vital signs.
- Carry out nursing procedures in patients' homes, including wound dressing and management, removal of stitches, venepuncture, urinary catheter changes, and other related tasks.
- Ensure continuity of treatment plans for home care patients.
- Maintain accurate and detailed records of procedures performed and medications administered.
- Collaborate closely with other healthcare professionals to provide holistic care for patients.
Requirements:
- Possess a Diploma in Nursing with valid SNB registration
