Responsibilities:

• Handle all driving assignments carefully and efficiently
• Ensure all transport requests are handled timely
• Responsible for the maintenance of the transport vehicle include cleaning of vehicle etc.
• Ensure the safety precaution are taken in the course of work
• Transport elderly clients safely to and from and within the Centre
• Transport elderly clients for hospitals/polyclinics medical appointments
• Transport staff to attend work-related functions such as hospital/home visits of clients, funeral wakes, corporate events
• Cover other drivers’ duties when they are away

Requirements:

• Secondary education with valid driving license
• Possess a good driving record with minimum of 2 years driving experience, preferably handicapped van
• Physically fit
• Interested in elderly and community work
• Able to converse with elderly

Job Description:

• Establish Individual Care Plan (ICP) of clients and implement intervention plans
• Assist in identifying clients with early onset of medical conditions
• Conduct group/individual activities and provide care for the clients
• Guide/assist clients in the activities such as exercises, games, handicrafts, outings
• Assist with the transportation and escorting the clients to, from and within the centre
• Serve meals and provide personal hygiene care
• Measure and monitor clients' vital signs such as blood pressure , heart/pulse/respiratory rate

Requirements:

• Certificate/ training in Healthcare preferred
• Relevant working experience, preferably in social service sector or healthcare industry an advantage

• Provide case management and counselling support.
• Assess client's needs or presenting issues, where necessary provide timely interventions and actions for desired outcomes.
• Participate in case management meetings.
• Collaborating closely with various department staff and agencies to meet the psycho-social emotional needs of clients to ensure a safe home or social environment for holistic care.
• Coordination of AIC or MSF or social referrals for financial or psychosocial care services.

• Financial assistance – Application for grants, FA case report and financial counselling.

Job Requirements:

• Diploma in social service or related discipline, with a social service background

• Manage the day-to-day operations of the Day Care Centre include enquiries, feedback and incidents.
• Formulate and review the Standard Operation Procedure (SOP) regularly.
• Ensure there is compliance of the SOP/work processes and proper documentation required by the relevant authorities.
• Coordinate & collaborate with different services within the organisation to ensure smooth transition of patient care.
• Oversee clients' Individual Care Plan.
• Manage, develop, supervise a team of healthcare staff.
