Job Description :

• To work with medical social worker on referral cases
• Organize home visits and fieldwork under the guidance of MSW
• Provide administrative procedure to patients and their families.
• Help patients apply for financial assistance application and AIC Long Term Schemes
• Maintain an updated register and records of patients
• Respond to inquiries regarding referrals, services, and charges.
• Ensure timely and accurate confidential documentation of patient assessments, care plans, and referral statistics.
• Assist with data entry and filing

Job Requirement :

• Singaporeans only
• Min GCE O Level
• At least 2 years relevant healthcare industry experience
• Good interpersonal and problem-solving skills
• Proficient in English and able to converse in dialects
• 5 days work week
• A caring, positive and self-disciplined individual, ability to work independently as well as within a team.
• Proficient in MS Office
