As a Senior Medical Social Worker, you will be responsible for the following duties:
- Assess the psycho-social status of the patients and families to determine the appropriate care plans.
- Counsel and support the patients and families associated with the illness.
- Work closely with community partners for the welfare of the patients.
- Deliver public education talks.
- Maintain working relationship with other disciplinary teams on patient care plans.
- Work with other provider to provide appropriate services to ensure continuity of care.
- Provide training and guidance to juniors and new hires
- Share the sessions with stakeholders and other professionals.
- Maintain the right documentation for case management and audit purposes.
Requirements:
- Possess a minimum Degree in Social Work/ Post graduate diploma in social work or equivalent
