Job Responsibilities:
Coordinate and Schedule Diabetic Foot Screening Services, Diabetic Retinal Photography and Nurse Counselling Services
Assist in implementation and educating patients on Healthier SG and benefits
To work closely with the GPs to manage identified patients who need support and referral to community services
Tracks, Monitors and follows up on the quality of care provided by GPs and other Primary care professionals and to ensure maintenance of good clinical care
Assists GPs in extraction and submission of data and other relevant administrative work
Involve in the PCN planning team to ensure smooth coordination within PCN
Has experience conducting screening and provide comprehensive health education to patients with chronic conditions
Ability to assess patient’s medication compliance, social history, current lifestyle and dietary habits
Has the ability to provide good clinical judgement, provide patient care in line with work instructions and protocols
Assist in data collection for quality improvement projects
Willing to work on Saturdays when required
Perform any other task assigned by PCN leader or Supervisor
Job Requirements:
Diploma or degree in Nursing
Registered Nurse with the Singapore Nursing Board
Experience in mental health and chronic disease management will be an advantage
Experience in healthcare or social services
Good interpersonal and communication skills
Has a positive attitude and a good team player
Fluent in English, Chinese and a local dialect to communication with senior citizens
