Job Responsibilities:

• Conduct initial consultation with participants using data gathered from a digital health platform
• Create and implement personalized health plans that are aligned with lifestyle goal(s) set by a General Practitioner (GP) utilizing digital tools and resources
• Use data and analytics to track participant progress and adjust plans as needed
• Monitor participant adherence to the program and encourage continued participation
• Provide ongoing support and guidance to participants through virtual coaching sessions and personalized nudges
• Communicate effectively with participants via phone and nudges
• Maintain accurate and confidential client records
• Consolidate and routinely update the list of existing activities available within the Queenstown community
• Stay up-to-date with the latest digital health technologies and trends

Job Requirements:

• Possess a diploma equivalent to professional qualifications in Nursing, Social Work, Allied Health, Health Services Management, Health Management & Promotion, etc. Those with Nitec in Nursing and relevant clinic work experience will be considered.
• 1 - 3 years of experience in healthcare and/or community health settings in Singapore is preferred.
• Strong team player, with a natural ability to interact with patients/residents, healthcare team, and community partners of all levels.
• Self-motivated and able to work independently and manage a high volume of clients
• Strong communication and interpersonal skills. The ability to use local languages and dialects will be an added advantage
• Strong understanding of health and wellness principles and ability to apply them in a virtual setting
• Willingness to learn and adapt to new technology
• Proficiency in MS office, including Word, Excel, Powerpoint and Outlook
• Motivation interviewing and/or Health Coaching certification is a plus

Roles & Responsibilities:

• Conduct Work At Height inspections and safety assessment on rooftop to identify roof condition in the worksite and raise reports on follow-up actions including monitoring the compliance
• Perform Site survey for roof top and recommend safety control measures
• Will be expected to be trained and execute solar system basic engineering site survey
• Provide guidance and support to the organization to review and implement work at height control measures
• Perform site safety inspections and submit daily reports.
• Responsible to ensure safety compliance when performing works on roof
• Have knowledge in WSH Act and familiar with Design for Safety (DFS) requirements
• Review and acknowledgement of Method Statement, risk assessments, Fall prevention plan , safe work procedures and other related HSSE documentation.
• Responsible for coordinating all site inspection for work at height
• Work closely with relevant departments leads and provide HSSE advise
• Assist the organization to conduct internal Work At height Internal Trainings
• Perform any other ad-hoc matters assigned by Management / Reporting Officers

Requirements:

• WSQ Advance Certificate in Workplace Safety & Health
• Relevant Safety & Health Certificates, including Building Construction Safety Supervisor Certificate
• Manage Work At height
• WSQ Develop A Risk Management Implementation Plan
• Occupational First aid + AED
• Working-At-Height Rescue Course (WAH-R)
• Preferred electrical background required
• Minimum 5 years of relevant experience in the construction industry
• Preferred Rooftop experience in Work at height
• Familiar with work at height and risk management procedures
• Familiar with MS Office application
