We are looking for a committed Health Management Executive (‘HME’) who is passionate to reach out to the seniors to conduct community screening and outreach, monitor their health and enrol them into the health management plans.

Key Responsibilities:

· Provide health awareness and basic monitoring for seniors with chronic illnesses, including those identified to be at-risk and their caregivers.

· Perform basic health monitoring and keep track of seniors’ health plans and flag out non-compliance or other concerns to social workers / senior health management executives.

· Plan and conduct outreach to the community to promote wellness and health awareness.

· Manage volunteer and befrienders deployment and feedback.

· Perform community health screenings and strengthen collaboration with local organizations.

· Members recruitment and renewal.

· Review and submit Community Screening Tool (CST) forms.

· Plan, develop and manage engagement programs held at designated neighbourhood touchpoints for seniors and their caregivers.

Requirements

· Minimum Higher Diploma or Diploma certification in Gerontology or ITE NITEC in Community Care and Social Services, with 2 years of relevant experience.

· Resourceful and able to do public speaking when handling events and projects.

· Conversant in English and any 2nd language with ability to converse in local dialect would be an advantage for elderly clients

· Competency in Microsoft Office applications especially in MS PowerPoint and MS Publisher

· A good team-player with a passion to work with Seniors

· Good communication and interpersonal skills

Dental Assistant

Monday to Friday 10AM-7PM (half-day work on one day 2pm-7pm),

Saturday 10AM-5PM, 5.5 working day

Sunday off day

Responsible for assisting dentists, making appointments with customers, and handling basic customer information

Dental Clinic

Need to know basic computer skills, basic English conversation, dental assistant experience preferred

Responsibilities:

• Responsible for the quality of scanned medical records by performing the tasks of document preparation, scanning, indexing and quality control of the scanned medical records within stipulated timeframe.
• Ensure medical record sent for imaging are properly accounted for and assembled according to guidelines.
• Ensure all discrepancies are tracked and promptly followed up.
• This contract is renewable depending on candidate’s job performance and department needs.

Working hours:

• Monday to Thursday: 8.30am – 6.00pm
• Friday: 8.30am – 5.30pm

Requirements:

• Minimum GCE ‘O’/’A’Level, or ITE Certificates in any general discipline
• Candidates without relevant experience are welcome to apply as on-the-job training will be provided
• Knowledgeable with Microsoft Office and computer literacy
