Key Responsibilities:

Requirements:

As a member of management team to provide thought leadership in strengthening evidence-based quality person-centred care (PCC) nursing practices towards well-being and wellness of all profiles of persons living with dementia in the Lodge.

Responsibilities:

1. Provide visionary leadership in transforming and maintaining high standards in the delivery of evidence-based PCC dementia nursing care in accordance with MOH Enhanced Nursing Standard for Nursing Home (ENHS), Healthcare Service Act (HCSA), Singapore Nursing Board Standard of Practice & Code of Ethics and person-centred care principles.
2. Provide training, coaching, supervision and monitoring to build a capable and forward-looking team for high and consistent quality dementia care.
3. Provide leadership to the Domain Expert teams in reviewing SOPs for relevance and alignment to person-centred principles, facilitate and monitor the implementation of ENHS for consistent high-quality and safe person-centred nursing care.
4. Lead Quality Assurance team through structured audits, lead relevant Quality Improvement (QI) projects and on-going analysis of clinical data to improve quality and efficiency delivery of person-centred care.
5. Lead team in harnessing technology to enhance safety, productivity and deepen PCC nursing.

Requirements:

• A degree in nursing from an accredited University and an advanced diploma, post-graduate, post-basic nursing qualification
• Full registration with Singapore Nursing Board
• At least 10 years as Staff Nurse with 5 years of leadership experience in a nursing homes, inpatient hospice, and inpatient wards in both acute and community hospitals.

• Excellent communication and interpersonal skills
• Dynamic, resourceful, driven, visionary and able to work independently and within a team
• Experience with dementia population will be an added advantage

RESPONSIBILITIES

Quality and Safe Care

• Is responsible for the compliance of all nursing standards as stipulated by Ministry of Health and Singapore Nursing Board.
• Leads quality assurance activities to monitor and evaluate the quality and appropriateness of nursing care. Ensures quality audits and planned and conducted periodically. Follow up with areas of corrective improvements.
• Ensures written policies and procedures and work process are available and up to date to guide the provision of nursing care.

Care Delivery

• Monitor the smooth transition of the residents from acute to chronic care and back to the community, where possible.
• Direct care of the residents to ensure compliance to standards.

Staff Resources

• Ensures appropriate skills mix and adequate manpower resourcing for all shifts and types of services.

Staff Development

• Direct and facilitate staff development and competencies upgrading, in-house training, including latest nursing techniques; and implement new techniques or procedures where appropriate.
• Guide orientation/induction programmes and preceptorship for all new staff.
