Role Overview: As a Medical Social Worker at Ihsan Kidney Care, you will be responsible for delivering comprehensive care to patients in both the dialysis center and home dialysis settings. You will conduct assessments using tools such as the Bio-Psychological-Spiritual (BPSS) assessment and the Risk & Needs Matrix to guide patient management effectively. Your expertise in medical aspects and community dialysis settings will enable you to provide vital support to patients and their families.

Key Responsibilities:

• Engage with clients and conduct interviews to assess cases using various assessment tools, including BPSS and Risk & Needs Matrix.
• Develop holistic care plans to enhance patients' psychosocial functioning.
• Collaborate with caregivers to optimize care modalities and recommend respite when necessary.
• Administer a Job Placement Pilot Program tailored to IKC patients undergoing long-term dialysis.
• Conduct supportive counseling visits to dialysis centers, patients' homes, or hospitals.
• Advocate for patient dignity, confidentiality, and rights.
• Collaborate professionally with internal and external healthcare professionals.
• Network with government agencies, community organizations, and individuals to mobilize social resources.
• Facilitate support group programs for patients and caregivers.
• Maintain accurate records, including bio-data, session recordings, assessments, social reports, and patient feedback forms.

Requirements:

• Possess a Master's Degree or Graduate Diploma in Social Work.
• Minimum of 3 years of social work experience.
• Registered Social Worker (RSW) with the Singapore Association of Social Workers or eligible for RSW status.
• Proficiency in Microsoft Office (Word, Excel, PowerPoint).
• Ability to speak local dialects or Malay/Mandarin/Tamil strongly preferred.
• Comfortable working in a fast-paced, multi-disciplinary team environment.
• Strong knowledge of the health and social sectors in Singapore.
• Excellent communication and interpersonal skills.
• Capable of working independently and collaboratively.
• Experience in casework, counseling, social and financial assessment, care plan development, and liaison with government agencies.
• Competent in means testing and other relevant returns submission.
• Skilled in managing staff of all ages.

Job Summary:

As a Medical Devices Product Specialist, you will be a crucial member of the team, focusing on Neurological and Cardiovascular products.

Your primary responsibility is to serve as the expert in presenting, explaining, and promoting the technologies and specifications of medical devices to Direct Distributors and Medical Professionals. This role requires a keen understanding of the Neuro and Cardio sectors, with a future focus on targeting the APAC Market and global expansion after obtaining necessary certifications.

General Responsibilities:

1. Product Presentation and Demonstration: Present and explain the technologies and specifications of Neurological and Cardiovascular medical devices to Direct Distributors and Medical Professionals.
   Conduct product demonstrations, emphasizing features and benefits during presentations.
   
2. Market Expansion Strategy: Contribute to the development of market expansion strategies, with a particular focus on the APAC Market.
   Plan and execute initiatives to target global markets after obtaining necessary certifications.
   
3. Sales Support: Collaborate with the Head of Department to develop and implement sales plans for Neuro and Cardio products.
   Provide technical support to the sales team, assisting in achieving sales targets.
   
4. Continuous Learning and Improvement: Stay informed about industry trends, competitors, and emerging technologies in the Neurological and Cardiovascular medical device sectors.
   Continuously improve product knowledge and stay updated on relevant certifications.
   
5. Customer Relationship Management: Build and maintain strong relationships with Direct Distributors and Medical Professionals.

Functional Skills and Knowledge:

RESPONSIBILITIES

Our Medical Social Workers collaborate with a multi-disciplinary team of stakeholders to regularly review individual residents' medical, nursing, therapy, and social aspects as part of delivering person-centric care. They will be involved in the means-testing review, and provide fair payment proposals, financial application assistance, and any appropriate assistance based on the social & economic status of the family and/or resident.

Admissions

• Provide financial counseling upon admission
• Co-ordinate an assessment on site of the patient to assess nursing/rehab status/behavioral and any social concerns

Case Conference

• Review social aspects inward with regards to activity participation, visitors and befriending, financial consideration status, etc.
• Provide therapy/nursing/medical/social recommendations and support family and resident to complete home discharge or other care options
• Review supporting documents for referral to other care options and seek financial assistance for home equipping as recommended by the therapist where required
• Provide counseling to patients with psychosocial issues
• Facilitate family conferences for discussion on financial considerations, if necessary
• Work with the Nursing team to review care for residents and understand their social situation

Financial Assistance Schemes

• Work with resident or next-of-kin on financial considerations (e.g. Government Means Test subsidy)
• Review cases and facilitate meetings for Medifund cases 
• Apply for financial assistance (e.g. Means Test, Medifund, IDAPE, ELDERSHIELD, MFEC)

Reports

• Update residents’ profile and movement in database
• Review next-of-kin details and update residents’ social report
• Review residents accounts receivable status report 

Supervision/Development of team members

• Manage and supervise team members in case management team 
• Guide on the professional development of team members

• Degree in Social Work or related discipline
• At least 5 years of relevant experience
• Social work in healthcare background preferred
• Good interpersonal and problem-solving skills
• Caring, positive and self-disciplined individual
• Ability to work well in a team
