- To improve the collaborative partnership within the mental health ecosystem to achieve seamless process and structure in identification and referral of clients.
- Drive engagement and partnership with the hospitals and partners, to identify operational gaps and to develop suitable solutions to address those gaps.
- Develop referral and care processes to enable the flow and escalation of clients across care setting, for community partners to support clients with mental health conditions or complex needs.
- Conduct ongoing review to develop new model of care to support clients requiring mental health care support.
- Plan, develop, implement, monitor and review the ASCAT and Aftercare programmes, including enablers such as funding and processes to support the development of the community health ecosystem.
- Present findings and potential strategies to senior management, government stakeholders and industry players to secure buy-in for execution.
- Building capability of partners for assessment to identify both medical and behavioural needs.
- Formulate and implement plans to develop the capability of community partners in supporting clients with mental health needs.
- Establish a sustainable financing and resourcing framework.
- Responsible for the overall programmes’ key performance indicators, including the management and utilisation of the allocated budget.
- Prepare annual budget and programme progress updates.
- Analyse programme care model and to propose funding model at steady state.
- Manage and lead a group of programme managers.
- Provide supervision, guidance and advice to assigned team members.
- Develop platforms for discussion and cross-learning with staff on programme management.
Job Requirements:
- Bachelor Degree/Post Graduate Diploma/Professional Degree preferably in Social Work/Psychology/Counselling.
- At least 8 years’ experience, with at least 2 years in a managerial capacity, in healthcare or community care sectors preferred.
- Experience and knowledge in facilitation, design thinking, organization development preferred.
- Experience in community development and multi-agency work in healthcare/social care industry would be an advantage.
- Able to work well with groups from diverse backgrounds.
- Able to multi-task and possess strong analytical, problem solving and project management skills.
- A self-starter with strong learning ability, who is resilient, adaptable and able to manage change and ambiguity.
- Possess strong negotiation and communication skills.
