• Develop Individualised care plan base on client's needs with person-centred approach
• Oversee clients' medical / nursing needs and ensure service delivery according to service contract and maintains documentation accordingly
• Collaborate and co-operate with internal and external care team members for co-managed cases
• Conduct education and training sessions for caregivers and / or clients to promote person-centred care needs
• Conduct in-service training sessions to guide and support staff and volunteers
• Develop and review workflows or Standard Operating Procedures (SOP) to aid operation needs
• Recommend new strategies for service development and quality improvement / excellence to supervisor and / or manager
• Be a good team player, building positive influence to team and motivated to learn and grow towards self-excellence
• Perform any other duties as directed by supervisor and / or the management

• Diploma/Bachelor Degree in Nursing

Relevant Experience

• At least 2 years of relevant experience. Experience with community nursing is preferred
• Fresh graduates are welcome to apply
