- Conduct group activities for carers and/or persons with dementia.
- Engage with various stakeholders to ensure the smooth running of programme.
- Support outreach events and initiatives.
- Explore new avenues of collaboration with community partners and stakeholders.
- Provide case management support to carers of persons with dementia.
- Ensure proper documentation of client follow-ups.
- Update statistical data in a timely manner for funder reporting.
- Perform other duties as assigned.
Requirements
- Degree in Psychology, Social Work, Community Development, or a relevant discipline, with a minimum of 2 years of experience in community engagement and/or case management.
- Prior experience working with persons with dementia or caregivers is an added advantage.
- Ability to work on Saturdays.
- Self-motivated with a high level of initiative.
- Ability to plan and implement ideas effectively.
