As a Case Management Executive, you will be responsible for the following duties:
- Working with Medical Social Worker in assessing, evaluating, planning and coordinating resources and services for residents.
- Collaborating with multidisciplinary health service professionals to develop and implement care programs that ensure timely treatment and recovery for residents.
- Monitor the bed occupancy rate and ensure that it aligns with the admission Key Performance Indicators (KPIs) on a monthly basis.
- Analyzing trends and report any discrepancies or areas of concern to ensure optimal bed utilization.
- Keep accurate and up-to=date records of residents' data and information.
- Apply relevant financial schemes for residents and monitor the associated pay-outs on a monthly basis.
- Ensure that financial assistance is appropriately allocated and processed according to established guidelines.
- Monitoring the renewal status of residents' Household Means-Test to ensure it is updated before it's expiration.
- Coordinate with relevant parties to facilitate timely renewals.
- Prepare and compiling Medifund reports and documents required for submission to the Medifund Committee.
- Ensure all reports are accurate, complete and submitted within the required timelines.
- Addressing and resolve queries and concerns from residents and their next-of-kin in a timely and professional manner.
- Provide support and information to residents and families regarding services, policies and care plans.
- Assist with any additional case management tasks as assigned, ensuring all aspects of resident care and services are manged effectively.
Requirements:
- Diploma in Nursing, Social Work or other related disciplines.
- Minimum 1 year of experience in community healthcare settings in Singapore.
- Good knowledge of the geriatrics and community services.
- Strong communication skills (oral and written).
