Reporting to the Deputy Chairman, Medical Board (Continuity Care Disciplines), the Senior / Manager is responsible for strategic planning and driving the development of new care integration initiatives undertaken by the office. The Senior / Manager will also be responsible for seeking out appropriate community and industry partners in support of the development of new care integration initiatives and established programmes undertaken by the office. He/she will collaborate closely with internal stakeholders to communicate population health initiatives and corresponding changes in operational practices, to ensure maximal adoption within the institution.
The roles and responsibilities of the Senior / Manager includes:
- To engage CGH’s internal clinical and operational business units from SingHealth institutions and MOH/AIC to identify care integration needs, conceptualise, develop and implement new initiatives.
- To manage and guide fact-finding, population health database and predictive tool development to enable business units to develop appropriate actionable measures to right site patients to appropriate care setting.
- To monitor established programmes and to work closely with programme managers to ramp up programmes and ensure their continued relevance.
- To identify, engage and build strong partnerships with community healthcare services and partners including Primary Care, SACs, allied health and home nursing services and other relevant social services and community-based organizations.
- To facilitate building of capabilities of primary and community care partners through identification and implementation of key programmes and initiatives. To identify workable operational models to achieve eventual benefit of the patient.
- To identify, engage and build strong partnerships with appropriate and relevant industry partners in seeking novel and financially-viable options of implementing new care integration initiatives.
JOB REQUIREMENTS
a. Educational / Professional Qualifications:
- Bachelor Degree with at least 10 - 12 years relevant experience in healthcare
b. Relevant Experience
- Strong analytical skills with an understanding of financials would be an added advantage
- Excellent project management skills with experience in managing and supervising administrative projects
- Excellent written and verbal communication skills
- Able to work in a dynamic and fast paced environment.
