The Case Manager provides leadership and oversees the COMIT team to ensure service delivery is in accordance to the intended programme objectives and care model for COMIT programme as outlined under the terms & conditions of the Letter of Award/Addendum.

They maintain an overview of the service planning and implementation of the COMIT programme.
