RESPONSIBILITIES

You will be responsible for assisting with the onboarding of Nursing Home residents and ensuring that they continue to receive the quality of care as described in NTUC Health Care Philosophy and Service Standards.

Customer Journey Experience

• Act as the main liaison and single point of contact (Care Concierge) for next-of-kin of our nursing home residents.
• Engage caregivers and residents at the pre-admission stage to address any concerns or anxieties and to better understand the specific needs of the resident.
• Conduct pre-admission counselling sessions to help caregivers understand the fees, services provided, philosophy of care, living conditions and expectations of residents and next of kin.
• Receive the resident on the day of admission, conduct orientation and ensure that the resident settles in well to the new environment.
• Actively engage residents and caregivers throughout the period of stay and address any concerns that may arise by working together with the multidisciplinary care team.
• Be involved in multidisciplinary discussions as part of patient-centered/directed care planning by providing relevant information of residents' social background, preferences and habits.

Service Quality

• Help to develop and maintain important touchpoints for the resident journey from pre-admission to discharge.
• Assist in the measuring and monitoring of customer satisfaction via residents focus groups, periodic audits, residents satisfaction surveys, etc.
• Assist the manager in implementation of internal and external service recognition programs and events e.g. annual Service Excellence Award.
• Provide inputs during team meetings on service gaps based on the feedback received and propose service initiatives and training programmes for continuous improvements.

As a Care Concierge, you will be interacting with residents from all walks of life. Many of them have multiple medical conditions which may affect their behaviour and disposition. To succeed in this role, you must enjoy interacting with people from diverse backgrounds, be empathetic and be able to appreciate their point of view. This is not a desk-bound job and will require constantly being on the move. The joys of the job come in appreciation from residents and their next-of-kin as transitioning to a new living arrangement can be challenging. • Recognized diploma or degree,
• At least two years of work experience, preferably requiring interaction with a diversity of people

• People-oriented, willing to work the ground

We want to make a difference in our community by continuously exploring and expanding our services. We are recruiting dynamic and capable individuals for our Active Ageing Centres.

This role will perform data quality analysis, raise awareness and to establish and maintain standards, operational procedures and documentations. It is also required to conduct compliance checks to review and analyse the operation processes to ensure that the implemented controls and their outputs meet the requirements. Additionally, the Senior Associate is expected to visit the different AACs under NTUC Health for the adoption of the standards, data quality analysis and compliance review.

Job responsibilities:

• Systematically collect, process and perform analyses on large data sets to support decision-making in AAC operations and in meeting requirements.
• Use statistical tools and techniques to explore and analyse data, identifying patterns, relationships, and trends
• Prepare reports and presentations to communicate progress, insights and findings from the data to stakeholders, which can influence policy and decision-making processes;
• Collaboration working with other departments to drive and/or support the digital transformation journey.
• Design methodology for the review of compliance to AAC Operations Standard
  • Conduct compliance checks through interviews, observations and reviews of documentation, to ensure quality management measures are in place, effective and sustainable
  • Organize and conduct quality-related internal audits and coordinate external audits on AACs
• Develop roll-out implementation plans and materials
• Raise awareness on quality through various channels such as, but not limited, to organizing Quality Day events, issuance of quality newsletters or staff training
• Work with Centres'-In-Charge to understand ground issues and concerns, identify areas of improvements and develop improvement plans
• Compile, analyze data and findings and provide recommendations
• Establish operational procedures to improve consistency and efficiency in day-to-day processes
• May require to perform additional duties as requested

Technical qualifications

• Degree / Diploma in a relevant discipline
• Have a background in quality management, data analysis and / or service excellence programmes.
• Competency in data quality analysis

RESPONSIBILITIES

As we are growing our cluster support with the evolving EC model, we are looking for dynamic individuals to drive quality assurance, statutory compliance, and programme management in all Active Ageing Centres (AAC).

Quality Assurance And Compliance:

• Ensuring AACs comply with the funder requirements on service deliveries.
• Oversee the semi-annual AIC progress reporting, ensure correct reporting from AAC to funder.
• Monitor and maintain quality standards of the AACs including tracking, updating, and maintaining SOPs and Work Instructions under the framework of ISO 9001:2015 (Quality Management System Requirements).
• Oversee and manage regular quality surveillance, inspections, audits, and governance of Service Quality projects.
• Develop better practices, drive continuous improvement to a higher level of performance as well as implement sustainable business and operational strategies in line with the organization's growth.
• Engage all centers to implement and uphold the quality standards of the division.
• Act as the relevant contact for MOH Audit matters, providing communication to Ministry on improvement plans and escalation of issues.
• Share learnings and issues across various centers for learning purposes.
• AAC Data Protection Representative
• Conduct quality training, oversee and direct the development and implementation of quality and safety training strategies, initiatives, and programs for the department.
• May require to perform additional duties as requested

Incident Management:

• Establish a Quality Assurance Committee (QAC) to review and manage incidents/ data and other improvement plans.
• Oversee analysis, management, and reporting of incidents (Internal and external reporting)
• Oversee development and implementation of risk registry
• Collaborate with stakeholders to ensure that corresponding risk management measures are in place and sustainable.

• Bachelor's degree, preferably in Healthcare or Gerontology related discipline
• Preferably a minimum 3 years of Healthcare experience
• Minimum of 5 years in a supervisory role
• Minimum of 8 years experience in quality assurance, audit, knowledge management, and data analytics
• Experience in project management and/or process re-engineering using Lean and/or another quality improvement/innovation methodologies
• Good communication, interpersonal and people management skills
• Excellent analytical and problem-solving skills
• Detail-oriented
• Familiar with Google Workspace

• LEAN Six Sigma or equivalent process improvement experiences would be an advantage
