Job Description :
- To work with medical social worker on referral cases
- Organize home visits and fieldwork under the guidance of MSW
- Provide administrative procedure to patients and their families.
- Help patients apply for financial assistance application and AIC Long Term Schemes
- Maintain an updated register and records of patients
- Respond to inquiries regarding referrals, services, and charges.
- Ensure timely and accurate confidential documentation of patient assessments, care plans, and referral statistics.
- Assist with data entry and filing
Job Requirement :
- Min Diploma/Degreen in Social Work
- At least 1 years relevant healthcare industry experience
- Good interpersonal and problem-solving skills
- Proficient in English and able to converse in dialects or other languages
- 5 days work week
- A caring, positive and self-disciplined individual, ability to work independently as well as within a team.
- Enjoys in a Geriatric Care
