• To improve the collaborative partnership within the mental health ecosystem to achieve seamless process and structure in identification and referral of clients.

1. Drive engagement and partnership with the hospitals and partners, to identify operational gaps and to develop suitable solutions to address those gaps.
2. Develop referral and care processes to enable the flow and escalation of clients across care setting, for community partners to support clients with mental health conditions or complex needs.
3. Conduct ongoing review to develop new model of care to support clients requiring mental health care support.
4. Plan, develop, implement, monitor and review the ASCAT and Aftercare programmes, including enablers such as funding and processes to support the development of the community health ecosystem.
5. Present findings and potential strategies to senior management, government stakeholders and industry players to secure buy-in for execution.

• Building capability of partners for assessment to identify both medical and behavioural needs.
• Formulate and implement plans to develop the capability of community partners in supporting clients with mental health needs.
• Establish a sustainable financing and resourcing framework.

1. Responsible for the overall programmes’ key performance indicators, including the management and utilisation of the allocated budget.
2. Prepare annual budget and programme progress updates.
3. Analyse programme care model and to propose funding model at steady state.

• Manage and lead a group of programme managers.
• Provide supervision, guidance and advice to assigned team members.
• Develop platforms for discussion and cross-learning with staff on programme management.

Job Requirements:

• Bachelor Degree/Post Graduate Diploma/Professional Degree preferably in Social Work/Psychology/Counselling.
• At least 8 years’ experience, with at least 2 years in a managerial capacity, in healthcare or community care sectors preferred.
• Experience and knowledge in facilitation, design thinking, organization development preferred.
• Experience in community development and multi-agency work in healthcare/social care industry would be an advantage.
• Able to work well with groups from diverse backgrounds.
• Able to multi-task and possess strong analytical, problem solving and project management skills.
• A self-starter with strong learning ability, who is resilient, adaptable and able to manage change and ambiguity.
• Possess strong negotiation and communication skills.

As the programme manager in charge, you will play a key role in supporting and/or facilitating the implementation of national initiatives by the Ministry of Health (MOH) to keep Singaporeans healthy and to anchor effective chronic disease management in primary care. The role would require the incumbent to identify gaps, analyse information to find patterns and new opportunities, enhance existing service models and incorporating systems level structural changes needed so as to support a wider range of clients in the community.

Support the development of the programme:

• Apply structured methodology when working with the PCN team and stakeholders to analyse patterns, behaviour, systemic structures and deepen understanding of the challenges faced by PCNs.
• Through a structured approach, he/she will need to learn and/or understand concepts and match concepts to current state operating models, prioritise and recommend areas for change
• Have a strong overview of what these needs are by having e.g., an effective structure and process in that will tie in with the HealthierSG workstreams
• Able to propose, a coherent plan that can address PCNs' challenges by e.g., translating care models onto specific areas of need

Programme Evaluation

• Conduct evaluation and review of programme and projects. This includes conducting data analysis and research.
• Working with the Primary Care Leaders on the phases of change to the new model of care.
• Managing both the dependencies and the interfaces between projects that can span across different stakeholders in the primary care sector.
• Managing risks to the programme's successful outcomes including but not limited to grants processing, liaising with MOH and other AIC Divisions to ensure proper controls are put in place for fund administration.
• Initiating necessary actions and additional interventions whenever gaps in the programme are identified or where issues arise.
• Reporting the progress of the programme at regular intervals.

Job Requirements

• Degree holder in any discipline with minimum 7 years of relevant working experience (Experience in healthcare industry is a plus but not a must)Trained in systems thinking and experience in implementing and facilitating change would be an added advantage
