To partner with Therapeutic Programme Executive, Occupational Therapist, Physiotherapist and Psychologist in empowering Persons with Dementia(PWD) to live in wellness.

Responsibilities:

• Champion Person-Centred Care (PCC) nursing to provide dignified and safe care to person with dementia, empowering them to live in wellness
• As a key team memeber to an inter-disciplinary team to implement and enhance the evidence-based integrated PCC dementia care models
• Lead clinical and service quality management
• Grow to be a practitioner-trainer

Requirements:

• Degree or Diploma in Nursing from a recognized institution
• Registered with Singapore Nursing Board
• Deep interest to specialize and to lead dementia care
• Fresh graduates with deep interest to specialize in dementia care are welcome
• Excellent communication and interpersonal skills

• Strengthen Social Work Unit to deepen Person-centred admission to Long Term Residential Care and Community Care.
• Partner collaboratively as a team with multi-disciplinary professionals (Occupational Therapist, Physio-therapist, Therapeutic Programme Executive, Psychologist and Nursing staff ) to innovate, reframe dementia, facilitating holistic person-centred care for persons living with dementia and their caregivers to live a normalised life; empowered and actively engaged in the Lodge and in the community
• Networking with government, hospitals, community, social agencies and community partners to mobilise community resources to support persons living with dementia and cognitive impairments and their families.
• Empower family members living with people with dementia and mild cognitive impairments thru dyadic engagement.
• Contribute as a visionary member in the transformation of dementia care to reframe dementia as an unique phase of life.

Requirements:

• Degree in Social Work or Graduate Diploma in Social Work
• Registration with Singapore Association of Social Workers will be an added advantage
• Preferable with minimum 2 years of relevant experience
• Excellent conceptualization, administrative and writing skills
• A visionary, mature, caring, compassionate and pro-active team player
• Possess strong interpersonal skills

As the Head of Psycho-Social Care, you will provide visionary leadership in reshaping the care framework for persons with dementia and mild cognitive impairment (PMCI). You will drive the integration of evidence-based practices and develop sustainable, person-centred care models. This role involves working with multi-disciplinary teams, community partners, and caregivers to create an environment where dignity, wellness, and quality of life are at the forefront.

Key Responsibilities:

• Leadership & Strategy: Lead the strategic direction of the psycho-social care team, setting benchmarks and developing new care streams for PWD and PMCI in the community and the Lodge.
• Innovative Care Models: Synergize PCC with the Ability-Centered Care (ACC) model and mindfulness-based approaches, creating scalable, sustainable care solutions for evolving profiles of PWD and PMCI.
• Community Engagement: Engage and empower caregivers and community partners to co-create innovative care solutions, expanding services beyond Singapore through collaboration and knowledge exchange.
• Human Capital Development: Build and grow a high-performing, compassionate team that embodies AHL’s values of respect, responsibility, and resilience.
• Quality Assurance: Develop robust systems for continuous improvement in care delivery and ensure compliance with health, safety, and regulatory requirements.

Requirements:

• Certification as a trainer or in technology-enabled care models is an added advantage.
• A strong belief in person-centred care with a visionary and transformational leadership style.
