Job Description
Provides home nursing care.
Makes the initial home visit and performs a biopsychosocial and environmental assessment and conducts subsequent visits.
Develops a nursing care plan and implements nursing interventions as outlined in the care plan.
Initiates appropriate preventive and rehabilitative nursing procedures.
Provides nursing services requiring substantial and specialized nursing skill.
Provides patient and caregiver education.
Prepares clinical documentation.
Co-ordinates with other team members and updates them of changes in the patient’s status and needs.
Evaluates the patient’s response to care interventions on a regular basis and recommends revision of care plans as necessary.
Functions as the primary care manager for a given patient, where appropriate.
Assists the nurse-in-charge with stock inventory.
Requirements
Registered Nurse with Singapore Nursing Board
Basic computer skills for electronic health record documentation
Good communication & interpersonal skills
Good team player
