Support in implementing Dementia Singapore Ltd (DSG) palliative care initiatives. Roll out a palliative care framework that enables DSG to embrace palliative care principles and practices across the care services. Support DSG frontline staff to have palliative care capabilities and support in consolidating palliative care knowledge and resources.
Responsibilities:
- Assist the Team Lead in the development of a palliative care framework, its implementation and review of practices.
- Assist in the development of palliative care resources that benefit the community care sector at large.
- Provide clinical support to persons living with dementia and carers to improve their well-being, especially towards the advanced stage of dementia.
- Support DSG frontline staff in palliative care, especially in end-of-life care.
- Conduct case assessments and formulate Individualized Care Plans for persons with dementia and their carers.
- Provide interventions such as psychoeducation, behavioural management, meaningful engagement, supportive counselling, and service referral.
- Conduct Advance Care Planning (ACP) with persons with dementia and their carers.
- Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
- Provide case recordings and relevant documentation.
- Participate in team meetings and case conferences as required.
- Assist in preparing data and statistics for programme evaluation and reporting purposes.
- Any other duties as assigned.
Requirements:
- Degree in Allied Health / Social Work
- Prior working experience in community palliative care and dementia care.
- Familiar with the person-centred approach to dementia care.
- Experience in providing support to people with advanced dementia and their carers.
- Good team player and interpersonal skills.
- Good oral, written and communication skills.
- Able to work independently and with the team to achieve optimal care outcomes.
