Purpose of job
Provide leadership in developing, implementing, and evaluating evidence-based dementia and caregiver support programmes. Supervise the team’s clinical practice, lead outreach efforts, and engage stakeholders to ensure person-centric service delivery. This role supports the rollout of non-pharmacological services for persons with dementia, covering preventive to intervention work, in collaboration with partners and volunteers.
Key Responsibilities Areas
- Provide leadership in developing, implementing, and evaluating evidence-based dementia and caregiver programmes to ensure holistic and person-centric service delivery.
- Case Management of mental health and dementia-related cases, overseeing care coordination, case planning, psycho education, referrals, and multi-disciplinary interventions.
- Deliver psychotherapy, psychoeducation and care planning support for clients and caregivers after diagnosis from primary and tertiary healthcare settings
- Build and maintain networks with stakeholders, community partners, and agencies to enhance care support and service linkages.
- Supervise and guide the care team through individual and group supervision, ensuring adherence to service protocols and best practices.
- Provide clinical direction and monitor the care team’s practices to ensure high-quality service coordination and linkage.
- Develop and review care plans, workflows, communication processes, and service manuals for continuous service improvement.
- Oversee the training and professional development of the care team, monitoring competencies and performance to support growth.
- Prepare regular administrative reports in line with service requirements and programme goals.
- Plan, implement, and evaluate groupwork and outreach initiatives for clients and caregivers.
- Undertake any additional duties as assigned by the Head of Community Mental Health and the Executive Director.
Requirements
Experience:
- Relevant experience in mental health and dementia care, including case management, groupwork, programme development and community engagement
- Proven experience in supervising and leading teams to deliver high-quality services.
Qualifications:
- Graduate Diploma, Bachelor's, or Master's degree in Social Work, or
- Nursing or Relevant Allied Health Qualifications
Others:
Required travelling for home visits to different locations in service boundary.
Competencies
- Knowledge in case management for various mental illnesses, person-centric care, dementia, and disease management.
- Team player with strong interpersonal and communication skills.
- Preferably bilingual and knowledge of dialects will be an added advantage.
