THK EIPIC Centre involves structuring classes around small groups encompassing early childhood special education, social work and therapy support services to stimulate their adaptive functioning and school readiness skills.

Reports to the Senior Social Worker, HOD (Social Work).

Main Duties and Responsibilities:

1. Case Management Support

• Facilitate parents' understanding of programme and needs of child, and enrolment of students into EIPIC.
• Establish rapport, engage parents and conduct holistic bio-psycho-social assessment of child's and family's needs, risks and strengths
• Demonstrate knowledge of available community resources, evaluate and identify areas for follow up (e.g. referral to internal/external resources)

1. Documentation and administrative tasks

• Participate in team meetings/case discussion, take notes and maintain good record keeping
• Complete documentation (e.g. case recordings) accurately and promptly for other professionals who are working with child
• Maintain accurate records, prepare reports for governing agencies, collate information and submit government means-test applications
• Provide logistical and administrative support of programmes which are conducted by the centre/department.
• Any other duties assigned by the Senior social worker/HOD.

• Minimum Diploma in Social Work

The Senior Social Work Associate maintains professional relationships with service users, acting as a guide, a support, an advocator and intermediary.

• Undertake and write up assessments (reviewed by the Senior Social Worker) that meet specified standards and timescales;
• Conduct interviews with service users and their families to assess and review their situation;

You will be part of the team managing operations of the Active Ageing Centre (AAC) - a go-to point for seniors to build strong social connections, take part in recreational activities, access community health services, and contribute to the community.

General Operations

• Familiar with the objectives of Active Ageing Centre (AAC)
• Compliant to operation rules and guidelines for AAC, ensure safety of elderly and staff
• Promote racial/religious harmony amongst the seniors
• Assist in care of the seniors, observe and monitor their health and well-being
• Work closely with supervisor to:
• oversee compliance in operations, directives, KPI and all work processes within assigned centres/programmes/projects

RESPONSIBILITIES

Through our Active Ageing Centres, we engage the seniors within the clusters and support them by reaching out to understand their needs, curating programmes that cater to the needs of active seniors, and befriending isolated and vulnerable seniors.

As a Programme Associate, you play an important role in sharing our goal in the work of the AAC and assisting the Centre Manager in fulfilling the objectives of the AAC and meeting the targets set by the management and funders. You shall also look out for vulnerable seniors and support their needs.

The duties of a Programme Associate are as follows:

• Promote NTUC Health Active Ageing Centres' eldercare services and programmes.
• Conduct/support active ageing programmes and encourage active participation with seniors. The services include: conducting community screening assessments, organising and conducting active ageing programmes, provide befriending services (conducting home visits) and monitoring their vital signs.
• Where required, coordinate care and support services through internal referral or to external agencies.
• Provide support to seniors at outreach events and programmes.
• Carry out administrative functions such as data entry, record keeping and report submissions in a prompt and accurate manner.
• May be required to perform additional duties and work over the weekends or after office hours.
