RESPONSIBILITIES

You will be part of the Community Case Management Service team, providing support to vulnerable seniors and their caregivers who face difficulties in managing their multiple health and social care needs. Key responsibilities:

• Provide intake and triage services for members of the community;
• Casework, counselling and service coordination for seniors and caregivers;
• Conduct psychosocial assessment and coordinate interventions with healthcare and community professionals to support seniors and caregivers;

RESPONSIBILITIES

The role will be part of the Family Support Services-Families for Life team playing a trainer, delivering evidence-based parenting and marriage programmes to parents and couples. It works collaboratively with schools and community partners in planning of these programmes as required, and will be involved in various community outreach efforts.

Roles & Responsibilities:

1. Case Management:

• Manage 12 to 15 parenting consultation cases per month
• Provide counselling, programme, and case management services, including information & referral for identified clients
• Conduct client assessment and intervention plan based on programme requirements
• Participate in networking and case management meetings, if needed

Job Purpose

Reporting to Head (People & Organisation Development, P&OD), you will be responsible for leading talent acquisition strategies and drive end-to-end recruitment activities in Care Corner. Working in partnership with all service points/departments, you will manage an efficient and robust hiring approach to secure a strong talent pipeline with the capability to achieve the organisation mission and goals. RESPONSIBILITIES

The role will be part of the Family Support Services-Families for Life team that delivers evidence-based parenting and marriage programmes. The role will work with trainers, school and community partners, in planning and promoting these programmes to support parents, newlyweds and married couples. It will also provide administrative and logistical support to ensure that these programmes run smoothly.

The Snr PE will lead a team of Programme Executives, and will be overall in-charge over outreach initiatives to key community stakeholders.

Roles & Responsibilities:

1. Work collaboratively with school and community partners in planning and promoting the programme to support couples, parents and families in building their resilience capacity.

• Provide administrative and logistical support in programmes and services to meet clients needs
• Carry out administrative and logistical requirements of agencys programmes
• Manage end-to-end operations from registration to execution and post programme review and reporting

2. Assist in planning and execution of agencys programmes

• Manage partnerships with schools and community partners in planning and promoting programmes to service users
• Provide support to the social worker/ counsellor/s as assigned by the supervisor

3. Outreach to community and to community partners

• Lead outreach initiatives, sharing of parenting and marriage resources and recruiting parents and couples to participate in the programmes

4. Record-keeping duties and administrative work

• Prepare and review monthly reports
• Review programme SOPs on a regular basis
• Maintain accurately and update programme data for report compilation and analysis
• Maintain database administration

5. Supervision and Management

• Supervise a team of Programme Executives (PE)
• Manage manpower resources of PE team and ensure fair allocation of tasks to ensure programme delivery

Requirements:

• Degree/Diploma
• 5 years work experiences in social service or education or training
• Candidates with less than 5 years of experience are welcomed to apply and will be considered for Executive level
• Experience in programme administration, front-line client facing work and managing partnerships
• Experience in leading or supervising a team
• Passion for parenting work to strengthen family life
• Intermediate proficiency with MS Office application, specifically Excel, Power Point presentation, MS Forms
• Proficiency in using MS Team/ Zoom to host seminars and do presentation and hosting seminars
