The Care Coordinator is responsible for providing support to the clinical team in the

to resolve personal, social, emotional, and financial issues.

He/ She will provide service coordination management, administrative support,

and operations improvement. Care Coordinator will need to work collaboratively

with colleagues from other departments to ensure a seamless client experience for

our service users.

Job Responsibilities

• Assist in initial assessment of client/caregiver needs with basic case management under the direction of a case manager/ social worker
• Ensure client and caregiver’s ability to cope by helping to streamline and coordinate the range of community services available
• Assist client/caregiver with appropriate information and referral assistance
• Conduct joint home visits with a case manager/ social worker or the multidisciplinary team where necessary
• Participate in case discussion with the clinical team
• Consolidate, prepare, track relevant monthly reports and statistics

Job Requirements

• Minimum Diploma in any discipline, preferably in healthcare or related discipline
• Team player with good interpersonal and communication skills
• Patient, gentle disposition with empathy and strong listening skills
• Able to converse in English and at least one other local language is preferred
• Must be able to commute between our 2 centres at Redhill and Boon Lay

To provide effective and efficient physiotherapy sessions and develop innovative solutions to meet the client needs, in line with best practice. He / She will work with a multidisciplinary team to ensure the coordination, development and delivery of a quality, client-centered therapy service.

The physiotherapist will also be required to support other departments in providing clinical support where necessary, to ensure a seamless journey for our clients and their caregivers.

Responsibilities

Clinical Duties

• Conduct accurate and holistic physiotherapy specific assessment for clients during admission, periodic care plan reviews and discharge
• Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making
• Formulate person centered and SMART goals in collaboration with clients and relevant stakeholders
• Design plans and conduct physiotherapy interventions for clients
• Document assessment results, therapy plans, interventions, goals and outcomes measures according to guidelines
• Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholders
• Recommend appropriate assistive device and equipment as needed
• Formulate discharge and continuity of care plans based on clients’ performance in consultation with relevant stakeholders
• Facilitate community integration through collaboration with other community partners as required
• Empower clients and caregivers with methods to enhance health, mobility and functions through self-management strategies
• Participate in case discussion with the care team using a multidisciplinary, collaborative approach

Requirements

• Degree or Diploma in Physiotherapy
• AHPC Full Registration
• Able to converse in English and other languages
• Minimum 2 years relevant experience preferably with neuro-rehabilitation
• Dynamic and proactive personality with good public relation and communication skills
• Passionate in serving the community

Beyond rehab, the Wellness Team plays a critical role in this continuum of care by creating a safe and supportive environment for stroke survivors and their families to enhance their physical, mental, social, and emotional health.

The Executive, Wellness & Community Health will play a key role in supporting the development and implementation of:

1. S3’s Wellness Programme
2. S3 Membership for Stroke Survivors
3. Ensuring the smooth coordination and management of volunteers.

• Curate, plan, and coordinate the schedule of both on-site and virtual Wellness sessions and events for Wellness clients.
• Manage the recruitment and onboarding of S3 members and wellness clients and caregivers.
• Acts as the point of contact for clients and caregivers
• Train and work with volunteers who can provide support during the Wellness sessions.
• Develop and programme suitable activities for each domain of Wellness e.g. Physical, Cognitive, Social, Learning, Events etc
• Sources and liaise with service providers on services for clients and caregivers
• Prepares paperwork including but not limited to onboarding recruitment, surveys, and programme reports.
• Assists to develop programme effectiveness surveys, compiles, and analyses the results and prepares reports as required.
• Organises and coordinates the logistics for programmes and events.

Requirements

• Minimally diploma in any discipline.
• Diploma in Social Science or Healthcare related course would be advantageous.
• Meticulous and organized.
• Ability to communicate effectively, possessing good stakeholder management skills.

• Carry out prescribed therapeutic exercises and activities as instructed by therapist
• Render physical assistance to therapist during therapy sessions
• Prepare client for rehabilitation and other activities
• Setting up and maintaining of equipment or logistics for therapy and programme activities
• Conduct group exercises and activities.
• Assist in development of new programmes.
• Assist with outreach programs to enrol new clients or volunteers.
• Responsible for housekeeping at the centre and safety of the activity areas
• Perform basic data entry and administrative task

Requirements:

• Minimum GCE ‘O’ or ‘N’ levels with Higher Certificate in Healthcare Support (Therapy Support) or equivalent
• Minimum 2 years of experiences in similar position or in healthcare sector

Duties and Responsibilities:

• Conduct speech and language specific assessments for clients during admission and periodic reviews
• Design plans and conduct speech and language interventions for clients using evidence-based practices
• Provide individual and group interventions for clients
• Assist in the discharge planning and care plan for clients
• Participate in case discussion with care team
• Assist in the development and review of rehabilitation and wellness programmes for clients
• Training and supervision of therapy assistants

Job Requirements:

• Degree in Speech Therapy.
• Minimum 2 years of relevant experience
• Full registration with Allied Health Professions Council.
• A valid certification in Basic Cardiac Life Support with AED.

The Manager will play a key role in overseeing and leading the development, implementation, and evaluation of:

• S3’s upcoming AAC@Boon Lay
• S3’s Wellness Programme and Membership
• S3’s Stroke Prevention and Outreach efforts

The Manager can expect to:

• Lead the Wellness coordinators to operationalise the Wellness and Active Ageing services programme offerings
• Develop partnerships with service providers and partners
• Coordinate and plan programmes
• Provide a seamless onboarding experience for clients/caregivers to the Wellness Programme
• Plan, budget and oversee the development of AAC programmes and activities for seniors at Boon Lay

Requirements:

• Bachelor's degree in any discipline with at least 5 - 8 years of relevant experience preferably in a Healthcare environment
• Meticulous and organized
