As a member of the CGH care team, you will work closely with nurses, and other healthcare professionals to deliver quality care to patients. You will perform basic nursing care activities for patients and assist in ward administrative functions to facilitate smooth operations in inpatient care, but not limited to the following areas:

Patient Care
• Record vital signs, assist in wound dressing, perform blood glucose testing and specimens collection, assist patients with general hygiene and in other nursing procedures as required
• Perform admission, transfer and discharge of patients
• Perform any other supportive duties as required

Visitor / Patient Services
• Engage patients and visitors to understand their needs

Admin / Operation support
• Perform ward administration work such as coordination of various services, inventory management, and participate in quality improvement initiatives

Job Requirements
• Minimum GCE ‘N’ or ‘O’ Levels
• Prior working experience as a paramedic or in a ward setting will be advantageous
• Good interpersonal skills, able to communicate with people at all levels effectively
• Independent, highly organized, able to work in a fast-paced environment
• Proficient in MS Office applications
• Able to perform rotating shifts, including weekends and public holidays
• Training will be provided

As Assistant Care Coordinator, Primary Care Partnerships in RHS Office, you are responsible for the following areas:

Job Responsibilities

• Responsible for the coordination of care of residents under the care of the General Practitioners (GPs) and Nurse Practitioner (NP).
• Support GPs on primary care initiatives and/or Primary Care Network (PCN) related matters.
• Data entry into database including tracking and coordination to ensure that patients follow the care plan for preventive and chronic diseases as laid out by the GPs and NP.
• Participate in committees or projects to improve care processes related to preventive care or chronic disease management.
• Promote health screening and wellness to the patients through care planning and coordination of care in the community.
• Provide health coaching to PCN patients and work closely with PCN GPs to improve clinical outcome.
• Coordinate social and community referrals eg. Active ageing centre AACs, IRMS services, etc.
• Provide support in quality improvement projects.
• Perform any other administrative duties as assigned by supervisors.

Job Requirements

• Possess a Diploma (Preferably in Health or Social related)
• At least 1 – 2 years of relevant work experience (Fresh Graduate are welcome to apply)
• Strong communication and personal engagement capabilities of older adult in the community
• Able to multi-task and work independently and as a team

Job Description:

• Provide post discharge follow-up via a coordinated process of care planning & coordination, telecommunication and telehealth monitoring under the supervision of a Nurse Clinician.
• Manage Medical Tier 2 and Tier 3 patients and provide post discharge phone calls.
