- Uphold and support Centre’s philosophy in infant care and development.

- Conduct professional practices in line with guidelines set in ECDA’s Early Years Development Framework.

- Work in partnership with parents to foster holistic and positive development in infants.

- Design and implement lessons plans / activities based on Centre’s curriculum.

- Reflect and document evaluations of lessons.

- Monitor & document growth and development progress of infants in prescribed forms.

- Be actively involved in the infant care routines so as to be able to coordinate and assist in the management of workflow.

- Provide mentorship to Infant Care teachers to maintain quality curriculum and infant care services.

Job Summary

1. Part of the centralised referral team that is responsible for:

• Management of referrals for all Senior Care Centres (SCCs),
• Implementation of Admission processes centrally and at SCCs,
• Support other functions of SASC HQ where necessary.

Principal Responsibilities

1. Liaise with referral Source/ Agency for referrals related matters such as obtaining sufficient information (past medical history, social reports etc) for referral screening, other contacts for non-responsive contacts.
2. Screen referrals for right siting of services according to the criteria
3. Contact clients or caregivers to clarify services and needs which includes financial assistance, transport, escort etc.
4. Schedule appointments for admission assessment accordingly and update centres of the referral status.
5. Conduct pre-admission counseling which includes explaining the service terms and fees to the clients or caregivers, clarifying their concerns if any.
6. Raise referrals to other service providers.
7. Provide reports to the management on referrals status and centre vacancies for monitoring and reporting purposes.
8. Support the onboarding of new staff in centre-based administrative processes.
9. Support other functions of SASC HQ where necessary.
10. Carry out other responsibilities and duties as assigned from time to time, e.g. coverage duties at centres and HQ.

Job Description

• Ensure daily needs of residents are well taken care of
• Motivate residents through their day to day interaction
• Oversee security screening to ensure contrabands are not brought into the house.
• Facilitate the process of residents checking in and out of the halfway house on a daily basis and provide daily updates on the headcount and other activities/incidents (if any) throughout the tour of duty. Any unusual movements should be highlighted and reported to the relevant parties.
• Refer residents’ issues to Social Worker when necessary
• Assist in institutional programmes such as outings (optional)
• Oversee Halfway House estate management
• Assist in monitoring of workplace safety and health standards.
• Ensure the overall safety and well-being of the residents during the programmes and activities that are carried out in-house and externally.
• To ensure that there is seamless transition during the handing and taking over of shift duties. The jobholder is also responsible in ensuring that all outstanding matters have been clearly communicated to the next duty personnel and are promptly followed up with.
• Work closely with Programme/Social Work team in the case management of residents, including monitoring and maintaining proper records on the progress, movement, and other relevant information pertaining to the residents.
• Assist in handling any grievance procedure and carrying out the preliminary investigation of incidents and/or disciplinary issues (when assigned).
• Identify any defect(s) or work(s) to be carried out pertaining to the halfway house’s estate management issues during a daily tour of duty and to feedback to the Facility Department for follow up.
