We are currently recruiting an experienced Student Care Teacher for our MindSpace Centre located at Sembawang Shopping Centre. You will be given the opportunity to grow together with the business and be rewarded commensurately for your efforts.

Job Responsibilities

• Deliver MindChamps enrichment lessons and programmes for primary school students
• Provide guidance to primary school students to ensure completion of their daily homework and other school assignments
• Maintain accurate and timely student records and update parents regularly on their child’s educational and developmental needs/progress
• Oversee the preparation of teaching resources and materials
• Manage the overall appearance and learning environment of classrooms
• Maintain and build strong relationships with students and parents
• Serve as an inspiring role model with a creative spirit and a champion (positive) mindset
• Ensure safety of students at all times within the premises
• Manage other ad-hoc duties as assigned by Centre Director

Job Requisites

• Diploma or degree in a relevant discipline with minimum 2 years teaching experience
• Passion and proven track record in teaching primary school students
• Familiarity with MOE primary school syllabus
• Excellent communication and classroom management skills
• Enthusiastic, lively and energetic with a positive growth mindset
• Creative and nurturing; passionate about connecting and engaging with children
• Excellent service attitude with good ability to manage enquiries from parents

MAJOR DUTIES AND RESPONSIBILITIES

• Be certified and trained in skills as an ACP Facilitator and ACP Trainer.
• Facilitate ACP discussions with patients/individuals, families and NHS.
• Document these discussions and the wishes of the patient/individual in the appropriate forms (these may be hard copies or electronic forms).
• Update and liaise with primary care physicians on patient’s advance care plan.
• Coordinate between patients/individuals, relatives, and healthcare team for ACP facilitation and to follow-up with the necessary appointments for them.
• Mentor, train and guide junior ACP Facilitators in ACP case management, documentation and communication skills.
• Develop and improve ACP training curriculum and programmes e.g. Advocacy, General ACP and Preferred Plan of Care (PPC) Facilitation courses.
• Be involved in grief bereavement work and provide psychosocial support to patients and where appropriate, follow up with referrals to spiritual and/or psychiatric support.
• Develop and be involved in the spiritual care work and support for ACP patients and their family members.

GENERAL

• Support or conduct ACP awareness talks, road shows and training.
• Work in collaboration with key stakeholders in the evaluation and quality improvement processes for the ACP program.
• Participate in conducting audits, research and surveys related to ACP. Where necessary, staff will be nominated as co-investigators of research projects and are expected to be involved in data collection, abstract and paper writing etc.

JOB REQUIREMENTS

• A Basic Diploma with 5 to 6 years of working experience
• Strong communication skills, motivated, resourceful and a good team player
• Experience in ACP facilitations for 2 to 3 years in any setting

PROFESSIONAL LICENCE

• Nursing or allied health (social work etc.) background preferred.

ABNORMAL PHYSICAL WORKING CONDITION

• Staff may be required to travel to patients’ homes, other step-down care units or other locations as needed to conduct ACP discussions.
• Staff may be exposed to patients and their family members of various psychological and emotional states and who possess varying personality traits.

RESPONSIBILITIES

• Assist doctors and registered nurses in simple procedures
• Documenting, monitoring and recording observations of residents and their vital signs and reporting changes in condition to the registered nurse

• Escort residents for medical appointments
• Guide residents to participate in development and recreational programmes
• Provide general supervision of residents

No specific qualification required.

You will be considered for the position of Nursing Aide with at least a qualification in any of the following:

• NITEC in Community Care & Social Services;
• NITEC in Nursing;
• HMI Institute of Health Sciences' Certificate in Healthcare Support
• WSQ Certificate in nursing care-related courses and / or relevant work experience in nursing care

• Ensure that the Centre's nursing services meet and adhere to regulations and licensing requirements.
• Educate and supervise the conduct of correct nursing practices and high service standards.
• Develop and lead a team of competent and dedicated staff.
• Share leadership in developing a SNM Senior Care Centre that is known for its professionalism, compassion and effectiveness.
• Work with Head of Centre to develop and implement clinical skills development programmes for nursing staff.
• Conduct orientation and clinical skills development programmes for nursing staff.
• Conducts continuous assessment and evaluation of staff’s clinical performance and professional conduct.
• Act as a role model in clinical practice. Initiates clinical quality improvement and research activities to enhance client's care management and improve practices.
• Acts as a resource person for all categories of staff within the Centre.

Job Requirements:

• Candidate must possess at least Diploma in Nursing
• Possess State Registered Nurse license
• Valid Practising License with Singapore Nursing Board
• At least 2 Year(s) of working experience in a Senior Care Centre or at least 4 years or more of nursing experience as a Staff Nurse.
• Possess interpersonal skills and passionate about working with the elderly.
