We are looking for a committed Health Management Executive (‘HME’) who is passionate to reach out to the seniors to conduct community screening and outreach, monitor their health and enrol them into the health management plans.

Key Responsibilities:

· Provide health awareness and basic monitoring for seniors with chronic illnesses, including those identified to be at-risk and their caregivers.

· Perform basic health monitoring and keep track of seniors’ health plans and flag out non-compliance or other concerns to social workers / senior health management executives.

· Plan and conduct outreach to the community to promote wellness and health awareness.

· Manage volunteer and befrienders deployment and feedback.

· Perform community health screenings and strengthen collaboration with local organizations.

· Members recruitment and renewal.

· Review and submit Community Screening Tool (CST) forms.

· Plan, develop and manage engagement programs held at designated neighbourhood touchpoints for seniors and their caregivers.

Requirements

· Minimum Higher Diploma or Diploma certification in Gerontology or ITE NITEC in Community Care and Social Services, with 2 years of relevant experience.

· Resourceful and able to do public speaking when handling events and projects.

· Conversant in English and any 2nd language with ability to converse in local dialect would be an advantage for elderly clients

· Competency in Microsoft Office applications especially in MS PowerPoint and MS Publisher

· A good team-player with a passion to work with Seniors

· Good communication and interpersonal skills

Job Summary:

The Senior Care Coordinator will lead a dedicated team in providing comprehensive care coordination and case management services to patients across inpatient, outpatient, and community care settings. This role involves planning and implementing policies and procedures that support caregivers, enabling them to better assist patients.

Responsibilities and Duties:

Specific Responsibilities:

- Conduct home visits to deliver psychoeducation, support, and care coordination services to clients and caregivers.

- Assess patients' medical, psychological, social, environmental, and vocational needs.

- Collaborate effectively with a multidisciplinary team, including allied health professionals, doctors, and nurses.

- Provide psychological expertise during multidisciplinary meetings.

- Facilitate training sessions for team members and caregivers as needed.

- Execute projects and responsibilities assigned by the supervisor or Head of Department.

General Responsibilities:

- Develop and implement care management tools to enhance patient coordination.

- Identify areas for improvement within the department and hospital, recommending actionable changes.

- Assist in preparing reports and maintaining necessary statistics.

Job Requirement:

- Registered Nurse with valid certification from the Singapore Nursing Board.

- Possess a Registered Mental Nurse (RMN) or Advanced Diploma in Mental Health, or relevant qualifications in social work, counseling, or psychology.

- Minimum of 3 to 5 years of experience in psychiatric settings (e.g., Community Psychiatric Clinic, Institute of Mental Health) preferred.

- Strong passion for community work and supporting individuals with mental health disorders.

- Excellent organizational, interpersonal, communication, and writing skills.

- Proficient in computer literacy, including MS Office and other relevant software.

- Familiarity with the healthcare environment and mental health services.

- Ability to multi-task, work independently, and collaborate effectively within a team.

As a Patient Care Ambassador, you will be a key member of our Clinic and Medical Centre, being the face of the Medical Centre, as well as handling administrative tasks and contributing to the smooth operation of our facility.

Role and Responsibilities:

• Manage administrative tasks such as patient registration, appointment scheduling, and maintaining accurate patient records.
• Operate the clinic CMS (Clinical Management System) for patient registration, data entry, retrieval of medical records, and generate reports based on test results for doctor’s review
• Handle billing and cashiering duties, including processing payments and issuing invoices or receipts.
• Introduce and explain medical services and pricing to patients in a professional manner, seeking opportunities to upsell when appropriate
• Assist with inventory management by monitoring and replenishing medical supplies and ensuring proper stock levels.
• Assist with providing basic medical screening procedures such as taking patient height/weight, administer vision tests, taking blood pressure etc. and phlebotomy (if qualified), or other clinical tasks.
• Ensure a clean and welcoming environment in the waiting area and examination rooms.
• Provide excellent customer service by greeting patients, answering enquiries and phone calls, and addressing concerns in a professional and empathetic manner.
• Collaborate with the Centre Manager and other healthcare professionals to coordinate patient care and facilitate the smooth flow of operations.
• Adhere to strict infection control protocols and maintain a safe and hygienic environment.
• Stay updated on clinic policies, procedures, and services to provide accurate and up-to-date information to patients.

• Diploma or Nitec with at least 2 years of prior experience in a related field
• Minimum of 2 years of relevant experience in a healthcare setting, preferably in a similar role
• Proven experience in performing clinic duties and administration of health screening tests
• Familiarity with operating a clinic CMS system and preparing medical reports
• Strong organizational and multitasking abilities to manage administrative tasks effectively.
• Excellent communication and interpersonal skills to interact with patients, colleagues, and other healthcare professionals.
• Attention to detail and a high level of accuracy in recording patient information and test results.
• Ability to work in a fast-paced environment and handle potentially stressful situations with professionalism and empathy.
