The Senior/ Physiotherapist develops and provides rehabitative care and services based on the care model, to residents of the Nursing Home(s). You will have opportunities to work with other healthcare professionals to design appropriate rehabilitation programmes for the residents of the nursing home. You may be involved in developing prevention and treatment programmes with inputs from the residents and may use advanced technology or robotics to relieve pain, improve muscle strength, joint range and mobility, increase exercise tolerance, and improve respiratory function. You will also provide caregiver education to facilitate residents’ recovery.

Clinical Care

• Assess, evaluate and identify rehabilitation needs of residents; formulate/develop and implement appropriate individualised physiotherapy programmes to facilitate, restore, improve or maintain movement/function.
• Evaluate, monitor and review treatment on a regular basis to document needs and progress of diets and ensure that treatment is appropriate and effective.
• Evaluate and prescribe assistive devices to improve function in activities of daily living as required by residents.
• Maintain good communication and liaise with various disciplines involved in the rehabilitation process and refer residents to respective professionals for consultation and appropriate intervention.

Training & Development

• Guide, supervise and train physiotherapy students, therapist assistants, caregivers and volunteers on their performance of duties and implementation of programmes, to ensure safe and effective treatment of residents.
• Provide training for caregivers in preparation for residents' discharge back into the home environment.

Administration

• Maintain database/records of all residents to provide evidence-based treatment with accurate and appropriate documentation.
• Ensure that proper documentation is made for all treatment rendered to residents.
• Any other duty as assigned by the Rehab Manager or Director of Nursing

Requirements:

• Diploma/ Degree in physiotherapy recognised by the Singapore Allied Health Professional Council.
• Preferably with 1 to 2 years of experience, with experience in rehab services and hospitals, care homes and rehab centres. Fully Registered Therapist with the Singapore Allied Health Professional Council preferred

• Plan and run physiotherapy services in the Senior Care Centre
• Conduct assessments for SCC clients during admission and periodic reviews.
• Institute an appropriate and effective client therapy treatment.
• Participate in multidisciplinary case discussions with the SCC care team.
• Contribute to formulation and follow-up of clients’ Individual Care Plan [ICP] to ensure holistic health and care coverage.
• Plan and conduct group therapy programme and activities for day care clients.
• Review and revise programme schedule when necessary to enhance quality of therapy program.
• Take charge of proper documentation of therapy records.
• Understand dementia issues and know how to manage clients who display behavioural problems [associated with onset of dementia].
• Interact to foster close and warm relationships with clients’ families and caregivers; and to exhibit with sincerity the care and respect of clients.
• Take charge of other duties which the Centre Manager may assign from time to time.

Job Requirements and Qualifications:

• Bachelor degree or Diploma in Physiotherapy
• Qualified and registered with AHPC [Allied Health Professions Council].
• Minimum 3 years of working experience in a hospital or eldercare institution; preference is for a candidate with working experience in a community setting.
• Good communication and interpersonal skills.
• Possess a passionate disposition to interact with frail seniors.

Responsibilities

• Assess patients in a logical and sequential manner, generating and testing a hypothesis
• Analyse patients' problems directly related to the conditions referred, demonstrating sound clinical reasoning
• Plan and implement treatment, reflecting current available scientific evidence, ensuring patient safety and effectiveness of intervention
• Monitor patients' progress throughout treatment, prepare and maintain records and reports as appropriate, ensuring adherence to confidentiality requirements.
• Participate as an active member of the care team via attending relevant ward/unit rounds, clinics, meetings, and in research and work improvement activities.
• Contribute to achievement of departmental action plan, collaborating with internal and external clients in a professional manner.
• Contribute to learning and growth through active participation in departmental clinical education and clinical training activities.
• Supervise and facilitate learning of support staff/students/junior staff/peers, as required.
• Oversee Therapy Assistants on performance duties to ensure safe and effective treatment.

Requirements

• Preferably Diploma/Bachelor/Masters/Doctoral Degree in Physiotherapy
• Attain and meet all requirements to maintain registration with Allied Health Professions Council
• Good communication, interpersonal and patient handling skills preferred

Key Requirements

• Degree in Physiotherapy

• AHPC registered or eligible for registration

• At least 5 years of experience in Physiotherapy and allied fields

• Knowledge and experience of a wide range of musculoskeletal assessments and

treatment approaches for a wide range of clients with musculoskeletal conditions. Exposure to clients with rehabilitation and chronic condition management an advantage.
