You will oversee and supervise the performance of tasks in care plans by care team members and provides input in developing and reviewing care plans with the social service and/or healthcare professionals. You are also the primary assessor for admissions and referrals to the Senior Care Centre and the primary reviewer for care plans proposed by the care team and the rehab team.

Key Responsibilities:

Requirements

· Possess a degree in social work.

· Minimum 3 years of relevant experience in supervisory work managing an allied health team, or managing a centre.

· Experience in medical social work and familiarity with schemes such as Medifund (MF), Senior Mobility Fund (SMF), Medication Assistance Fund (MAF), Financial Assistance Scheme (FAS) will be an advantage.

· Fully conversant in English and Mandarin, or any 2nd and 3rd language will be an advantage due to clients being seniors.
