Opportunity to perform high-throughut chemical screens on cell lines and patient-derived tumor cells. We are looking to hire someone who is interested in doing wet lab experiments. As Assistant Care Coordinator, Primary Care Partnerships in RHS Office, you are responsible for the following areas:

Job Responsibilities

• Responsible for the coordination of care of residents under the care of the General Practitioners (GPs) and Nurse Practitioner (NP).
• Support GPs on primary care initiatives and/or Primary Care Network (PCN) related matters.
• Data entry into database including tracking and coordination to ensure that patients follow the care plan for preventive and chronic diseases as laid out by the GPs and NP.
• Participate in committees or projects to improve care processes related to preventive care or chronic disease management.
• Promote health screening and wellness to the patients through care planning and coordination of care in the community.
• Provide health coaching to PCN patients and work closely with PCN GPs to improve clinical outcome.
• Coordinate social and community referrals eg. Active ageing centre AACs, IRMS services, etc.
• Provide support in quality improvement projects.
• Perform any other administrative duties as assigned by supervisors.

Job Requirements

Job Description:

As an Assistant Care Coordinator in CareHub, you will be required to:

· Provide case management via a coordinated process of care planning and coordination, conduct regular phone call follow up and telehealth monitoring under the supervision of a Care Coordinator.

   Handle hazardous materials and chemicals safely, with proper disposal practices.
   Assist in the maintenance of lab documentation and compliance with regulations.
   
7. Inventory and Supply Management:Monitor inventory levels of laboratory supplies, chemicals, and equipment.
   Order new supplies when needed and manage stock to avoid shortages.
   

Skills and Qualifications:

• Educational Requirements: Minimum high school graduate in science stream. or equivalent is typically required; however, a postsecondary degree or certification in a scientific field (e.g., biology, chemistry, or laboratory technology) may be preferred. Retired personnal are welcome too.
• Technical Skills: Familiarity with laboratory instruments and techniques is important. Basic computer skills for data entry and analysis are also necessary.
• Attention to Detail: Accurate data recording and adherence to protocols are essential.
