Job Description

• Station in various community health posts, where patients/ residents are cared for within their community
• Triage and assess patient's medical-nursing, psycho-social, functional status and daily activity needs, as well as existing support system upon service enrolment
• Perform health coaching within the areas of health promotion, illness prevention and/or chronic disease management

Job Requirements

• Degree or equivalent professional qualifications in Nursing, Social Work or Allied Health profession.
